Organizes and maintains records on each patient and updates information on an ongoing basis, in accordance with Company policy and HIPAA regulations.

Plans and executes patient home visits to further promote patient loyalty to CHS.

Accomplishes established Patient Advocacy department program goals of Patient Retention and New Patient admissions to service.

Participates in weekly conference calls with Patient Advocacy staff, Blood Product Specialists, Patient Care Coordinators and the Director of the Patient Advocacy Department.

Provides Director with written and verbal reports.

Proactively advises and updates Director of all work related travel and function plans.

Evaluates current and future advocacy and individual service needs for existing or potential hemophilia patients, chapters, and HTC's.

Monitors and assists patients/family in exploring timely options with all insurance related issues, utilizing existing and available insurance options.

Plans and executes community events and functions with the goal of educating existing and potentially new patients to CHS programs and services.

Participates in and provides continuing education programs for referral sources and/or their support staff.

Attends all local and national hemophilia-related functions as a CHS Patient Advocate representative.

Demonstrates an understanding of and full compliance with all legal and budgetary constraints, relating to the Patient Advocate positions.

Becomes knowledgeable and familiar with the carious Company policies, procedures and resources including quality assurance, contractual agreements, reimbursement policies, infusion center connections, local and national corporate networks, etc.

Initiates and maintains existing and new relationships throughout the hemophilia community including, but not limited to state and local non-profit organizations and foundations, peer support groups, community organizations and HTC's.

Understands that the Patient Advocate is part of a multidisciplinary team of clinical and non-clinical staff, and endeavors to facilitate communication between all Company and referral source team members in an effort to maximize positive patient outcomes.

Performs other related duties as directed by supervisor.

Job Requirements

Education or experience equivalent to an Associate degree is required. 

At least one year experience in the hemophilia community is preferred.

Valid driver's license with appropriate insurance coverage.
